Service Development Manager – Specialist Home Care - City & County
Salary: £38,000 - £42,000
Location: Central & Southeast
Base: Home based role with travel to clients and branch locations
Summary
The main purpose of the position is to grow the hours of the specialist care business by winning and mobilising new contracts with Integrated Care Boards and other NHS commissioners

What you’ll do
You will have the responsibility of maintaining existing relationships with key stakeholders including Integrated Care Boards & Case Management companies.
You will be expected to ‘win’ new referrals and manage the onward discharge and mobilisation of the service user’s care programme which will be based at their home.
You will be responsible for the timely acknowledgement of referrals and creation of quotations along with mobilisation plans. Throughout the mobilisation pathway you will play a key role in ensuring that all elements from recruitment, training, care planning, risk assessments and financial agreements have been completed and are in place for the package of care to begin and for it to be handed over to the operational team.

What you need
Ideally you will have up-to-date knowledge of the specialist care environment, including regulatory requirements. Evidence of building strong and effective external relationships with customers, and the ability to build internal relationships and agree activities with colleagues to achieve an overall goal, along with persuasion skills to ensure timely completion.
You will have evidence of effective project management, planning and coordination skills. A Health and Social Care Level 5 or equivalent would be desirable.
If you have a genuine empathy with the care sector and care services. A motivation to do the right thing and a passion to ensure high quality care and high-quality outcomes for clients this is the role for you.
